Irritable Bowel Syndrome and Probiotics (What’s the Lowdown?)

Irritable Bowel Syndrome (IBS) is a common condition affecting millions of Australians, disrupting their daily lives. There are three main types of IBS: constipation-dominant, diarrhea-dominant, and mixed (the names are quite self-explanatory). In addition to constipation or diarrhea, all types share common symptoms like bloating, pain, and flatulence.

Our gut relies on a complex community of bacteria (called the microbiota) to help digest food. Without them, digestion wouldn’t be possible. In fact, there are more bacterial cells in our body than human cells! However, these bacterial cells are tiny, so they only make up about 200 grams of an average person’s weight. This vast bacterial community plays a crucial role in various bodily functions, including nutrient absorption, immune system regulation, and even mood.

We now know that certain gut microbiota compositions are linked to IBS. Studies have shown that individuals with IBS often have an imbalance in their gut bacteria, with lower levels of beneficial bacteria and higher levels of potentially harmful ones. Since probiotics can promote positive changes in gut flora, it seemed logical that they might help with IBS symptoms. However, until recently, there wasn’t strong evidence to support this. This inconsistency might be because different studies used different probiotic strains, dosages, and durations. As a result, doctors were hesitant to recommend probiotics for IBS on a widespread basis.

Recently, the World Gastroenterology Organisation (WGO) reviewed the effects of specific probiotics on IBS symptoms. Based on their findings, they issued a statement expressing more confidence in certain probiotics for IBS. The WGO focused on well-designed studies that used standardised probiotic strains and evaluated their impact on various IBS symptoms.

The WGO found good evidence supporting three specific strains and one multi-strain preparation. Let’s delve into the scientific names (brace yourselves!):

  • Bifidobacterium bifidum MIMBb75: This strain improves overall IBS symptoms and quality of life. Studies suggest it may help regulate gut motility, reducing both constipation and diarrhea.
  • Lactobacillus plantarum 299v (DSM 9843): This one helps reduce the severity of abdominal pain and bloating. It’s believed to work by modulating gut inflammation and reducing sensitivity in the digestive system.
  • B longum 35624: This strain, along with a multi-strain product containing L rhamnosus GG, L rhamnosus LC705, Propionibacterium freudenreichii ssp shermanii JS DSM 7067, and B animalis ssp lactis B012 DSM 15954, improves quality of life and anxiety related to digestive issues. These strains may work synergistically to improve gut barrier function, reduce inflammation, and promote a healthier gut environment.

Important Note: These probiotics can be beneficial for any IBS subtype except the multi-strain mix, which is most effective for the diarrhea-dominant subtype.

So, what can you do with this information? Before buying probiotics, check the label to ensure they contain only the strains mentioned above or a combination of these strains if you’re looking for the benefits offered by the multi-strain product. This increases your chances of experiencing the benefits. Additionally, discuss probiotic use with your doctor, especially if you have any underlying health conditions or are taking medications.

Living with IBS can be challenging. The unpredictable nature of symptoms can significantly impact daily life. Fortunately, there are steps you can take to manage your IBS and improve your overall well-being.

Dietary Management: Certain foods can trigger IBS symptoms in some individuals. Identifying and avoiding these food triggers can be a significant step in managing your IBS. A low FODMAP diet may be helpful in identifying potential triggers. This diet involves eliminating certain fermentable carbohydrates for a period and then gradually reintroducing them to see which ones cause problems.

Stress Management: Stress can worsen IBS symptoms. Techniques like mindfulness meditation, yoga, and deep breathing exercises can help manage stress and potentially reduce IBS flares.

Regular Exercise: Regular physical activity can improve gut motility and reduce stress, both of which can be beneficial for IBS management. Aim for at least 30 minutes of moderate-intensity exercise most days of the week.
